* Clinical Symptoms/Indications: Document any tenderness, pain, skin dimpling, or swelling.
* Previous History of Breast Cancer: Note any relevant past diagnoses. 
* Family History: Include information about parents, siblings, and grandparents with breast cancer, as this helps establish risk factors.
* Changes in Shape or Size: Detail any alterations in the breast(s).
* Thickening or Lumps: Specify any thickening, lumps, or lumpy areas, including differentiating between cysts and solid masses.
* Follow-Up for Calcifications: Mention if this is for monitoring previously identified calcifications.
